The Operating Room No Show Impact Estimator translates a surgical no-show or same-day nonattendance rate into expected unused case slots and reserved room time. It is designed for schedule planning when cases have protected room time that may be difficult to refill at short notice.

The estimate helps quantify the operational effect of attendance assumptions without assigning a dollar value to a canceled case. You can use it to compare baseline and improvement scenarios, estimate room-hours exposed to no-shows, or test how a different attendance rate changes expected completed cases. It does not predict which individual patients will miss appointments.

Assumptions: The calculation uses expected values, so the missed-case result can be fractional. It assumes reserved time is not backfilled after a no-show.

What the result means

Use the result as an operational planning estimate based on the values entered, not as a clinical, legal, or regulatory determination.

Actual performance can differ because demand, case mix, staffing, downtime, priorities, and local workflows vary.

Given:

  • 24 scheduled cases
  • No-show rate = 4%
  • Average reserved room time = 150 minutes

Calculation:
Expected no-shows = 24 × 0.04 = 0.96 cases. Unused minutes = 0.96 × 150 = 144 minutes. Unused hours = 144 ÷ 60 = 2.4 hours. Expected attended cases = 24 − 0.96 = 23.04.

Result:
2.4 expected unused room-hours.

Interpretation: Across many comparable scheduling periods, a 4% no-show assumption corresponds to roughly 2.4 hours of reserved OR time not used if missed slots cannot be filled.

Why can the estimated number of no-shows be fractional?

The calculator reports an expected value, not a prediction for one specific day. Over many periods, fractional expected cases are useful for planning averages.

Should cancellations be counted as no-shows?

Only if your operational definition treats them the same way and the slot cannot be recovered. For cleaner analysis, use the same definition as the source of your no-show rate.

What if the room time can be backfilled?

Then the result overstates truly lost room time. You can interpret it as time initially exposed to the no-show, or adjust the reserved-time assumption to reflect typical recovery.

Does this estimate patient risk or clinical urgency?

No. It only estimates scheduling and room-time impact from an attendance rate.

How can I compare an intervention?

Run the calculator once with the baseline no-show rate and again with the expected post-intervention rate, then compare unused room-hours and expected attended cases.
